INSTRUCTIONS FOR ORDERING

The online store catalog is in Microsoft Excel format. If you do not have Excel installed on your computer, a viewer can be downloaded here.

To place an order, you can call the Wellness Center or simply download and fill out an order form. Fax the form to 503-717-1378 or email it to yarrow@seasurf.net. Additonally, a paper copy of the store catalog and an order form is available at the Wellness Center.

Below is an example of how to fill out the form:
